What You Need To Open An Account

Personal Accounts

To be provided:

  • One of the following Identification:
    • Drivers License
    • State ID
    • Proof of Address if application address doesn’t match ID provided

Business Accounts

Sole Proprietor

To be provided:

  • Assumed Name Certificate and application - When the business name does not include the customers first and last name, an assumed name certificate is needed. Can get at the Circuit Clerk office in county primarily doing business in. Will be posted in newspaper.
  • EIN if applicable

Partnership

To be provided:

  • Partnership Agreement
  • Assumed Name Certificate and application (if applicable) - When the business name does not include the last name of all partners. County Document or Sec of State document
  • EIN
  • Beneficial Ownership
  • Meeting Minutes

Limited Liability Company (LLC)

To be provided:

  • Articles of Organization
  • EIN - EIN can be used for an LLC or Social Security Number if LLC is only owned by one person.
  • Example: Smith Electric LLC, Joe Smith
  • If using Social Security Number use alternate name on owner's CIF just like a Sole Proprietorship
  • Beneficial Ownership
  • Meeting Minutes
  • Operating Agreement- Who are managing members or just members

Corporation

To be provided:

  • Articles of Incorporation
  • EIN
  • Beneficial Ownership
  • Meeting Minutes (to show officers elected)

Public & State, Non-for-Profit, Estates, and Trust Accounts

Public & State

To be provided:

  • Articles of Incorporation or Association
  • Charter Bylaws (if available) - defining the organization, authority and responsibilities
  • EIN
  • Meeting Minutes

Association

To be provided:

  • Articles of Association
  • EIN
  • Meeting Minutes

Non-for-Profit

To be provided:

Follow above guidelines for Corporation or Association. They may not have outside documents, but we would need letter of purpose to state how long account will be open for and the intent of the account along with who as authority to do what.

Estate Accounts

To be provided:

  • Letters of Office - a document issued by the courts if the decedent' s estate is more than $100,000 in value. This Document also states whom is designated to act upon the deceased's behalf (Executor or Administrator) in order to disburse the assets, etc. (A will does not meet this requirement)
    Or
    Small Estate Affidavit - a notarized document if the decedent's estate is less than $100,000 in value. This Document also states whom is designated to act upon the deceased's behalf in order to disburse the assets, etc. (A will does not meet this requirement)
  • Death Certificate
  • EIN Number
  • Valid ID of Estate Representative

Trust Accounts with an EIN Number

When the Grantor(s) of the Trust are deceased, the Trust agreement will designate who the successor Trustee(s) is.

To be provided:

  • Death Certificate of the last deceased Grantor
  • EIN Number of the Trust
  • Valid ID of the Successor Trustee(s)
